In some cases, we may link the information you provide in the survey with Personal Information in your Dwell Well account, such as your name or email address. When linked to Personal Information, your survey information becomes Personal Information too.

Photographs of Completed Services: If you are using Dwell Well to request services, Our Dwell Well Certified Professionals may take photographs of completed.

If you use the Dwell Well website as a service professional, you may give us:

Contact information and identifiers, such as your name, email address, mailing address, and phone number
Application information, if requesting consideration to be a Certified Dwell Well Professional, such as your experience, skills, eligibility to work, and availability
Photographs of yourself
Social Security number for background check. In addition, when third parties such as Good Hire conduct background checks on service professionals, those third parties share the results of the background check with Dwell Well.

HOW WE USE YOUR PERSONAL INFORMATION: We never require that this Personal Information be provided in order to gain access to our site, nor will we require visitors to disclose more Personal Information than is reasonably needed to participate in an activity on our site. Generally, we use the information we collect through this Site:

to provide the information, products and services you request;
